In the mission briefing you are told that the agent is being kept in the medical unit of the rehab center, so keep that in mind as your initial destination. Also, as you're playing through you'll be shown cutscenes of three different men in three different robes (light blue, light green and light pink). One of these three men is the target the agent will identify for you.
Immediately move left and into the dining area. Continue straight and through the double doors. Pass through the first door on your right and move down the stairs. Wait on the landing until you spot the guard below come and go.
Continue down the stairs and look around the corner. Wait in position until you spot an orderly coming your way (you may have to run upstairs to hide from the guard again, particularly if he's walking with the orderly). When you spot the orderly (guard free), arm the Syringe Sedative and keep watching. When he turns the corner quickly move behind him and sedate him with the sedative. Put on the Orderly Suit and collect the Stun Gun and Cell Key. Dump his body in the nearby bin and close the lid. Alternately, you could slip the Sedative in the orderly's coffee which he sips every time he passes through the break room across from the bin. Sometimes I would be seen doing this and sometimes I wouldn't. It all depends on the pattern of the guard, and the pattern is different between games. So, if you find yourself having troubles with one of the methods, try the other one, and always watch the movements of the people on your map, as it will make life a lot easier. Finally, if the therapist follows you down the stairs you're going to have to wait until he moves past you, snacks in the break room, then returns up the stairs before doing anything, or he'll spot you.
With the orderly dumped and you in his garb, move to the far end of the hall then up the stairs to the right. Move straight ahead and unlock the door next to the medical unit reception (to the right of the orange couch). You'll find yourself on a catwalk above a few cells. Head down the stairs and locate the cell with the red haired man with his head in his hands. Stand beneath the stairs until the other orderly enters the cell across the way, then enter the red head's cell. This is the undercover agent. Close the door behind you then speak to him. When the conversation is done, hit the "Sedate Agent" button.
The agent presents you with a picture of the target. While the picture doesn't help much, your map will, as the target is the one marked by the red indicator. You'll also notice two pulsing indicators on your map. These are the other two mafia men, each are optional targets, worth $50,000 each.
The target can change between games. Most often it was Lorenzo Lombardo, occasionally it was Rudy Menzana, and never was it Carmine DeSalvo (but it could be on your game). Below you will find the easiest way to kill all three. On easier difficulties you can take out all three by following the below instructions and easily get a Silent Assassin rating. On harder difficulties it's more difficult because the bodies that can be found count against your rating.

Lombardo is the light blue robed man. Backtrack through the medical unit (close the agent's cell door as you leave) and the basement hall to the top of the stairs at the other end. Move through the door at the top of the stairs, then through the door directly across from you. You will find yourself in the kitchen. If you get there and Lombardo is there with you, exit out and wait for him to leave. Once you're alone in the kitchen, locate the bottle of booze in the back right corner, next to the mop and bucket. Lombardo sips from it every time he enters the kitchen. Arm your Poison Syringe and dump it into the drink. Exit out.
Menzana is the pink robed hit man who also likes his hidden booze. Menzana's drink is hidden in a golden globe in the library, which is directly across from the rehab's main entrance.
Exit the kitchen and move right. Pass through the dining area, the look for the open door to your left. Enter the room and note the globe. Make sure the room is clear and no one is looking in on you, then open the globe and inject it with poison.
DeSalvo is the green robed mafia man who loves to lift weights. Exit the library and move though the double door to your left. Watch for an open door to your right leading to an area with weights and trees. If you spot DeSalvo already on the bench, count yourself lucky. If you don't, you're going to have to wait for him to get there. Once DeSalvo is in position lifting his weights, move behind him, as though you're going to spot him. Here you'll get the option to "Drop Weight". Make sure no one in looking from your left, and drop the weight. Quickly exit via the door you entered.
At the bottom of the path you'll find two buildings. The building to your left is the morgue. Enter and close the door behind you. Walk over to the body of the "dead" agent. Stand in position and you'll get the "Revive Agent" prompt. Take it and wait for the agent to come to.
Simply move through the morgue door marked with the glowing "exit" sign.
