Run/Walk
You should also change the default Run/Walk to Run. I prefer it and I recommend it. You can walk by holding down the "Cancel" Button while moving. It's just easier that way.
Y-Axis
And, if you're like me, the first thing you need to do in any game is reverse the Y-Axis. To do so go to: options > controller > first person view, and then change it there.
Head Motion

By default, Head Motion is turned On. Head motion is something that has been in other "Silent Hill" games and is used as a way to let you know if there is something interesting nearby. When in 3rd Person view (for most of the game) pay attention to Henry's head. It'll move to one side or another if there is an item, memo, weapon, enemy, etc. nearby. This, however, makes for a weird First Person experience. However, the only times that you are in First Person are in your Apartment, so just get used to it.
Saving
There is only one place to save in the game: in your Apartment. On the PS2, I was given 30 save slots. Make use of each and every one of them. Don't keep re-saving over the same file. That way, if you get stuck or feel like you should have done a section better then you can jump back without losing too much time.
Weapons
There are many weapons in "SH4". In my opinion, the most useful weapon is the Rusty Axe, which you get in the Bar of the Building World. Until that time your best weapon is the Steel Pipe or Aluminum Bat. I didn't have much need for my Pistol or Revolver in this game because I was saving all my ammo for later. Well, you really don't need more than about 30 Bullets at the end, so feel free to use your Pistol all you want. At the end of the game I had about 180 extra bullets. There is also the Stun Gun which is very powerful but a tad slow. You can get that in Water Prison World.
To equip a weapon, pull up your Inventory, use your D-Pad to select a weapon then hit your "Equip" Button. To reload your Pistol or Revolver, highlight the Bullets in your Inventory and hit "Equip". This is especially useful to free up a slot in your inventory if you need to.
Fighting Enemies
Most of the Monsters in this game are more bark than bite, except for those darned Ghosts (see Sword of Obedience). I used my Axe almost exclusively throughout the game. I'd get near an enemy and start whacking. There was no reason to "Charge Up" with the Axe because it's a quick weapon. However, with the Steel Pipe and the Aluminum Bat it helps if you Charge Up your attack. To do so, make like you're attacking normally, but hold down the "Attack" button until you see your little meter next to your health meter charge up and flash. When it's flashing release the button to take a full swing.
Another useful tip is to learn how to dodge. While holding down the "Pose" button hit your "Dodge/Cancel" Button to dodge backward. This is especially useful for the Zombie Panthers.
Also be sure to "stomp" on an enemy when it is down to finish him off. You do this by getting near/over a downed enemy and hitting your "Attack" button. Otherwise, the enemy will rise back up. When it comes to Ghosts, your best bet is to simply avoid them.
Swords of Obedience
There are 5 Swords of Obedience throughout the game. That's rather convenient because there are 4 super-Ghosts that haunt you throughout the 2nd half of the game. Each of those 4 ghosts is somebody who has been killed earlier: Cynthia, Jasper, Andrew DeSalvo, and Richard Braintree. If you don't use a Sword of Obedience on these Ghosts when you can they'll haunt you throughout the game. I recommend pinning them down when you can.
Holy Candles and Saint Medallions
These items were a tad confusing to me at first, then I realized that they are almost exclusively for use in your apartment to exorcise the demons. The Holy Candles can be lit in your Apartment near a wall where there is Ghost coming through or any time you hear static and your screen turns a bit red. That means your Apartment is being overrun. Saint Medallions work much in the same way. When equipped, go up to a haunted area in your Apartment and you'll see the Medallion start to work. Saint Medallions do break after a while but there are many in the game.
You can use each of these items in regular gameplay as well, but I recommend saving them for your Apartment. Although, having a Saint Medallion equipped will give you early warning about a Ghost in regular gameplay.
Difficulty Setting
This guide is based on NORMAL DIFFICULTY.
Unlike past "Silent Hill" games, the difficulty setting in "SH4" only affects enemy AI, enemy aggressiveness, and your ability to inflict and take damage. In previous games, it meant the puzzles were different/more difficult. That is not the case here.
If you are new to "Silent Hills" choose Easy. If you want a challenging, yet doable, gameplay experience, choose Normal. If you are up for the challenge, choose Difficult.
Health
For the first half of this game you automatically heal when you go back to your Apartment. Therefore, any and all Health items that you find in the first half of the game should not be used unless absolutely necessary. Instead, store them in your chest.
Once you get to the second half of the game and you don't heal automatically, it is quite useful to have a stash of health in your chest. Use Health wisely and, if you're smart about Saving, you can always go back to a previous save and load up a better situation.
Map
"Silent Hill" maps are always some of the best and this is no different. The map tells you what doors you've checked, where you've been, and what floor you are on. This guide specifically refers to floors (1F, 2F, B1, etc.) in order to get you through the game.
Ranking
When you finally beat the game you'll get a ranking. One big star is 10% and the little stars are 1% each. You can get a maximum of 100%. You can get good rankings by killing lots of monsters, finding all the memo items, saving infrequently, and having low continues.

"Brand New Fear" - Beat the game on any difficulty setting then save the game file. Load "Brand New Fear."
Weapon - Chainsaw - Load "Brand New Fear" and when you get to Forest World (1st Time) go near Jasper's Car and look for the CHAINSAW near a tree stump.
Weapon - Submachine Gun - Get a 90% (9 big star) ranking or higher. Then, next time you play through you'll find the weapon in Room 102. This is an Eileen weapon.
Weapon - One Weapon Mode - This is challenge mode in which you are asked to play through the game with nothing but one weapon. First, you have to get a 100% (10 big star) ranking. Then, upon entering the Subway you can choose your favorite weapon. Get through the game with only this weapon and you'll unlock All Weapon Mode.
Weapon - All Weapon Mode - Beat One Weapon Mode, then in the Subway, you can choose all the weapons you want. Also, everytime you go back to the Apartment you'll find 10 Nutrition Drinks in the Fridge and 6 sets of Bullets in the Inventory Chest.
Costume - Eileen - Again, load "Brand New Fear." Play through and near the end when you can get into Eileen's Room (303) find the Nurse's Outfit. Keep it in your inventory, don't put it in the chest. If you save Eileen in the final boss fight, the costume will be available to you the next time you play through.
Costume - Cynthia - Get all four endings and save them on the same game save file. Make sure you have Eileen's costume. Choose Eileen's costume and when you play through again, you'll see the Cynthia has a different look.
